Before participating in high school athletics, students must register and have a complete account on PlanetHS. All athletic forms can be found on PlanetHS.com.
Athletic Forms must be completed after April 1st, 2021 to be valid for the 2021-22 school year. Students may pick up the current Pre-participation History Form and Pre-participation Physical Evaluation Form in the main office, athletic training room, or athletic director’s office. Once completed, forms must be uploaded onto PlanetHS.com along with other required documents. Students will also need to LINK ACCOUNTS with a parent account. Athletes are not cleared to participate until all forms are completed by both the parent and the student accounts.
If a student already has an account, please update the information and upload new documents to the account. If a student does not have an account, please “sign up” and follow the prompts to gain access to an account. Students will also need to link accounts with a parent account. If a student cannot remember a password, it can be reset by Coach Calvert.
